Not clear: Take her to a good physician. She needs a good workup.  There have good studies recently published which demonstrated that the taking of calcium and Vitamin D together did not improve bone health. That's why you have to do double-blind studies in medicine! Things that at first thought you might think "Sure, that will work." frequently do not!

Methotrexate effects: Muscle soreness is not a common side effect, but can happen.  You should get further laboratory work up to check your liver, kidney and muscle enzymes.  Yes, folate (folic acid) supplementation is recommended while taking methotrexate.

Many Possibilities: Depending on how long it's been going on, i would consider an evaluation by a spine specialist.  Sometimes it could be a muscle imbalance, early onset arthritis or scoliosis.  Also could be a pinched nerve/herniated disc.  Either way it might be good to figure it out and get appropriate treatment.  Regarding your fasting, you may choose to wait until after the month to see if the pain persists.

Extremities burning: Heavy exercise regimens will cause muscle inflammation which may present as a burning sensation.  Take a day or two away from your usual workouts, elevate your feet when at home, and if the pain persists, see your physician for an exam and testing...type 2 diabetes can also cause symptoms such as described.

No: its very hard to overdo on B12 and folate (folic acid) vitamins.  Your body gets rid of the extra.  Something else causing discomfort

K doesn't cause clot: While Vit K is needed for blood to clot, avoiding K doesn't reduce the risk of clots! The only reason to avoid is if you're on warfarin as K can interfere with it, but research shows being on some K actually stabilizes the effects of warfarin.
