Ciprofloxacin is an: Antibiotic that comes in an oral dose and a topical drops. If you are treating an external ear infection the topical medication is far superior because it produces a much higher concentration directly to the bacteria. If the canal is blocked with cerumen and debris the canal must be cleared first. The oral form  is not the preferred treatment for middle ear or external infections.

I assume you..: Are talking about an  external ear infection and referring to Cipro (ciprofloxacin) drops. If so, a 50:50 mixture of white vinegar and rubbing alcohol works well for killing bacteria and fungus. I recommend using it twice a day and flushing the canal with peroxide at the end of the day. If that doesn't improve things over a 3 to 4 day period you should see a doctor.

Usually enough: Usually a seven day course of Cipro (ciprofloxacin) would be enough to treat the infection if the cause is bacterial and it is sensitive to Cipro (ciprofloxacin). Since it did not respond to Biaxin, I encourage you to follow-up with your physician at the end of treatment to make sure things are clear. You may still have a little fluid left in the ear, but the redness and cloudiness should have cleared up.

A middle ear infection (the common type) does not respond to drops unless you have a hole or tube in your ear drum. Outer canal infections (swimmers ear) can sometimes respond to drops unless there is too much debris in the way.The date on the drug matters less than what you have.

Ear issues: It sounds like you should see your provider or an ear nose and throat specialist to try a different antibiotic and assess for whether antibiotic ear drops will help if the ear canal and ear itself are swollen as it appears you have possibly ruptured your ear drum but this is not always the case with drainage.

But, if this is bacterial, worsening suggests resistance and the need for a different antibiotic.
