Fibrocystic changes: Although you are not having a menstrual period, you are experiencing fibrocystic changes (especially if this occurs in both breasts).  This is common.  The pain can be aggravated by caffeine intake, hormone intake and stress. Decreasing all of these can help relieve the pain. Make sure you have had a mammogram and the results are negative.  Become familiar with breast changes with breast self exam.

Yes, generally. : Very common. Also at the time of the cycle.  Breast tenderness is very responsive to hormonal changes. These occur at he time of ovulation with a spike up then down in estrogen and again at the menses with a significant nadir in the estrogen and Progesterone levels. This alone dan cause he symptoms you describe without any seriousc.

Breast pain: At age 41, periods are frequently delayed, as the ovary requires more stimulation to produce mature eggs, delaying ovulation. Estrogen is therefore increased, producing breast pain.  Consult your gyn physician about the use of low dose oral bcp's, which stop ovarian hormone production and decrease both PMS, bleeding and breast tenderness.

Breast pain ?: The timing is a little short for breasts to be tender secondary to pregnancy, but with twin pregnancy, the estrogen/progesterone production may explain this.  Obtain a serum HCG test to be certain about pregnancy.
