Anatomy: Physical changes of the urethra, the tube that passes urine from bladder to outside, could cause this. This may or may not be of importance. Your primary care physician should be very helpful in knowing that your kidney function is normal. A urologist otherwise is the right specialist for this.

: If this only occurs in the first morning urination &amp; not during the rest of your bladder emptyings, it must be caused by some mucus or sperm that congealed overnight in your urethra. If it happens all the time, scarring or stricture of the urethra would be the likely cause. You would consult a urologist for such a condition.

Urethral Stricture: You may very well have some scarring in the urethra. This scarring can lead to other complications. If it does not quickly resolve then you most likely need to be seen by a urologist if not your family physician.

Wart? Fibroma?: Genital warts could do this, but usually not in urethra unless there also are external warts. Fibromas and other benign growths are possible. With altered urine stream, it is likely you have additional lesions deeper inside, or partial urethral blockage (stricture). You need to see a urologist about this. Don't manipulate the bump or otherwise try to treat it until then. Good luck!

Not usually: but some infections that might cause strictures might also cause abdominal pain, loss of appetite etc.  see an MD to evaluate

Congested prostate: Urethral strictures usually cause decreased flow, not usually painful unless associated with uti. I don't know how you can tell you have a congested prostate without symptoms. Weak stream and seminal fluid drainage during bowell movements can occur specially with constipation or severe straining with bm's. Voiding dysfunction can occur at times but more specific diagnosis will require an exam.

: It varies significantly among people. Capacity of female bladder is higher than male bladder but overflow incontinence is higher in female because the sphincter is stronger in male. Not uncommon, the urine may go back up to the kidneys (reflux) when the bladder is too distended if the vesicoureteral valve is relatively weaker than the bladder outlet sphincter.

Trauma: If he is having grossly visible blood in the urine he likely has damage to a kidney or other structures of the urinary tract.  He could have other injuries as well.  It may or may not be a severe injury, he should probably got to the ER and get evaluated.
