Rehabilitation: Many pts can recover or improve with physical therapy.  Carefully proper use of medications becomes even more important and a change or increase in level of assisted living is probably indicated.  Very sorry for your family.

Brainstem: Stroke in the area of the brain called brainstem can cause misalignment of the eye(s). There are parts of brainstem that control horizontal and vertical alignment. If damaged, they can cause hypo (one eye lower)- or hyper(one eye higher)- tropia.

Usually, with a: Complete history and physical.  Imaging will also likely be beneficial, especially a head mri.  Ct is much less helpful in cases such as this.

Please see your doctor.  A consultation with a neurologist (a physician specially trained in diseases of the brain and nerves) may be useful as well.

Any patient with dementia can be placed in a long-term care facility, not just patients with alzheimers. Contact social services in your area, or contact your doctor right away.

Definite risk: Heart surgery poses definite risk of brain injury or damage. Even absent embolization, brain dysfunction may occur after otherwise uncomplicated cardiac surgery. Since you are concerned, have a detailed discussion with both the surgeon and anesthesiologist before surgery. They should know your specific probabilities of these events.
