Diaphragm pain: Irritation of the diaphragm from abdominal or chest can be referred toor felt in the  shoulder. That is the muscle that expands the chest when you breathe. Coughing can be due to allergies, some medications, reflux, viral or bacterial illness among other things. I hope.

Lung Inflammation?: You may have inflammation in your lungs, and when you take a deep breath and your lungs expand, that is causing your discomfort.  Pleuritis is possible.  You may have a muscle sprain as well, and when you use your accessory muscles to breath, that are aches you.  See a dr. If it persists to get a proper diagnosis. Goodluck.

Would obviously also consider things in your upper belly on the left side, since your diaphragm, a dome shaped muscle under your lung, flattens during a deep breath, and pushes on the contents of your upper abdomen.

You will likely need an ekg (test that checks the electrical activity of your heart) and blood analysis. Several things: It could be from a cracked rib or inflamed rib joint. A blood clot caught in the lungs can do this. Irritation of the lung lining can do it too. If you are having trouble catching your breath, call your doctor or go to an urgent care or emergency room.

Pleurisy (Pleuritis): You are likely having pleurisy (pleuritis). Go see your doctor. Typical if this is all that it is, a prescription of nsaids like Ibuprofen will help take care of it. But you have to go and ensure there is nothing else that could be irritating the lining of your lungs like a collection, etc. This is likely unrelated to your chronic back pain.

Contributing: Probably can cause some restrictive lung problems eventually but your problem is probably due to your underlying lung disease primarily. Your posture can also eventually lead to circulatory problems and neuro symptoms in your arms (thoracic outlet syndrome). I suspect that your underlying lung  problems are severe. I wonder about a alpha-1=antitrypsin deficiency leading to severe emphysema.
