There is no way to say what your wife is doing is 'normal'. It is what it is. Indirectly: There are so many metabolic changes when blood sugar is so high, some of them may be capable of contributing to lowering your o2 sat. An example is dehydration caused by heavy urine output. This can cause mismatch between blood flow and oxygenation in the lungs, lowering o2 sat (probably not by a lot though).

No: Discomfort will stay in distribution of clot, and may see some swelling of the lower leg / ankle, some redness in the area of the pain, and the skin my get warmer than surrounding area, a little tense and shiny looking. If these occurring, or particularly if recent travel / immobility, should see dr for evaluation and rx for ultrasound of vessels to confirm / rule out.

Depends on origin: If a blood clot is in the leg (deep vein thrombosis), it can break loose and move towards the heart via normal venous blood flow. This clot can move through the right side of the heart and become lodged in the lung (pulmonary embolism)
a clot in the left side of the heart can break loose, leave the heart via the aorta, and move to the brain causing a stroke.

Prevent low sugar: How low is low? Any symptoms? You didn't list any medications or conditions that would explain why you have low sugar. Juice or small piece of hard candy, even glucose tablets, are often used to treat low sugars. But better to prevent event rather than keep treating. So talk to your doctor & get diagnosis.
