why do you twitch right before you fall asleep?

Part of sleeping: The falling sensation or twitch is known as a hypnagogic myoclonic twitch. As your muscles begin to relax in preparation for sleep, your brain senses these relaxation signals and misinterprets them, thinking you are falling down. The brain then sends signals to the muscles in your arms and legs in an attempt to jerk you back upright.

Sleep and jump: There is an area of the brain called the "reticular activating center.  This is sort of like a switch.  When you are drifting into sleep activating this switch changes one from the awake stage to the sleeping stage.  When the switch is thrown then one sometimes experiences the "jump" or twitch.

Myoclonus: This sudden jerking movement and or twitching body movements while falling sleep are quite normal and are known as myoclonic physiological jerking. It is a benign condition which does not need any treatment. If the twitching or jerking are localized just to one area of the body specific and repeatedly you may ask your PCP. Thanks

Inflammation : Inflammation of the nerves in your face, usually due to a virus.  This is treatable with course of steroids.

If a cyst increases : In size to the point that it compresses brain tissue adjacent to it, it may cause symptoms, sometimes seizures, the type of which depends on its location. Congenital choroidal  fissure cysts are benign, cerebro-spinal fluid-filled cysts that rarely cause smptoms or require surgical treatment. However, if these symptoms are new, see her neurologist to determine their significance.

RLS or meds: You may have restless leg syndrome or an electrolyte imbalance.  Both can cause what you describe as well as a malfunctioniung thyroid. Check with you md, who can treat either one.

???: Wake up right before you fall asleep?  People generally are awake before they fall asleep :-). If you mean that you wake up right after falling asleep, with a feeling like falling, this is a common occurrence and generally not a big concern. Just realize that you were startled and relax. If it occurs chronically, possibly see your doctor.

Palpitations: These are caused by many things. Stress, excitement, irritants both physical and chemical, pressure like a hiatal hernia or esophageal push or pull, and primary heart things, the causes are very many in number.

Sleep apnea: Thats one of the symptoms of sleep apnea. Make sure you use your mask. Some people depending on the severity of the apnea will use a dental appliance or have surgery. Make sure your CPAP has a humidifier on it as well

Cremasteric: Has to do with the changes in brain electricity which then can cause spasms in the leg muscles which affects the cremasteric nervous pathway. 
Not dangerous and common enough.
