why do ob docs measure cervical length?

Helps to check: Checking the cervical length give an approximation of how likely you are to still carry the pregnancy and not get close to delivery.  If the cervix length is 3cm or more, there is less likely a premature delivery. But this is only a guide. If you start having strong contractions this could change quickly. Also some women go the whole pregnancy with a cervical length of 2cm.

Yes: This is normal. You will have some thinning of the cervix as you get closer to the time of delivery. It is supposed to be closed and firm at 30 weeks, but it is normal if it gets somewhat shorter.

No: The cervix is a bit shortened and the progesterone is a tool to minimize the risk of further shortening.  It sounds like your doctor is on top of things and will watch you closely

my 1st pregnancy is fullterm with no complications[vaginal delivery] this is my 2nd pregnancy and during 20 week anatomy scan, my cervix length is 3cm. iam bit worried. my doc isn't concerned at all. do you think i need a transvaginal ultrasound?

: Trust your OB doctor, do not worry if your doctor doesn't. You had a normal first pregnancy so the anatomy of your cervix should be fine for the second one. Of course, a transvaginal US will give you a better measurement but you do not need to push for it if your OB doc thinks not necessarily. Good luck with your second baby.

Very real risk: The positive fetal fibronectin suggests that the risk is very real and treatment to prevent/stop preterm labor +/- steroids to hasten lung maturity should be considered based on your particular case. If one is available I would guess your OB wants to consult a maternal-fetal medicine specialist ( a doctor who specializes in high risk pregnancies). Best wishes to you and the baby. Yes: 3.2cm is more or less normal.   One study found that at 24 weeks gestation, the average cervical length is 3.5 cm.  We start to worry if it is shorter than 2.5cm as there is a 1/5 chance of pre-term delivery.    Best wishes!

At least 3 cm: Without knowing the outcome of a woman's previous reproductive history (what happened during/with any previous pregnancies) the length of the cervix should be at least 3 cm (1 1/2 inches).

Low Normal: A cut-off of 2.5cm is frequently used as the low-end of normal.  If the cervical length is less than 2.3, there is about a 20% chance of delivery prior to 34 weeks.   Best wishes!
