Reflex: We all have reflex reactions to stress; this, apparently, is yours. While there are far worse reflex reactions, injuring your own arm is not a good thing.  Why not carry a rubber ball or small stuffed animal or something similar, and grip that instead?

Several things: This is one of those problems where a visit to your doctor is necessary to figure out what's going on.  Only after a thorough evaluation, including examination and possibly labs and other tests, can your doctor correctly diagnose you and treat you effectively.

Always..: Assume you have done some significant damage unless it is a very, very superficial laceration.  Your best bet is to have a medical person look at, and advise you asap.

Laceration.  : Well, you definitely need a tetanus shot.  Outside of that, if it's been less than six hours at this point, stitches may be beneficial for healing and ultimate cosmetic result.  We tend not to stitch closed lacerations that have remained open more than six hours because it increases the chance of the cut becoming infected.

hyperpigmantation under arm bit. it resolved completely after using dectarine solution for nail! now no dactarine sol in our market what to use pls?

Hyper pigmentation: Did you mean dactarin? I am not familiar with a "dectarine solution" for nails. Dactarin - or Miconazole - is an anti fungal agent and if that is what you have been using, if the etiology of your pigmentation is fungal, it might well have been effective. There are other causes of pigmentation, however, one being acanthosis nigricans - which would be unlikely at your age. Consult a dermatologist.

Rabbit: Rabbits generally do not carry tetanus, however it would not be a bad idea if your last tetanus vaccine was in the last 10 years to get another one.

Needs to be seen: That should be evaluated by a ER physician/hand surgeon.  If the nail isn't secured back down underneath the fold of skin at the base of the nail it can cause healing issues

No: Terbinafine itself could not cause swelling or skin infection.  The cephalexin is an antibiotic which is used to treat bacterial skin infections.  The fungal infection and the bacterial infection are likely not related.  Was the fungal infection of the finger or toe nails?  Sometimes very bad fungal infections can leave breaks in the skin - this can allow bacteria to get inside and cause infection

Wrist injury: I would not expect your entire right upper extremity to hurt so much, so that makes me wonder about a possible infection. Normally one would get a tetanus shot every ten years. However, tetanus shot should be considered if none within 5 years for particularly dirty wound. What kind of wound care have you had?

Yes: If you received the tetanus shot documented 5-6 years ago, it is fine. However, you dont say when you got the injury and it sounds like it could be infected. If the puncture is more than 24 hours ago, I would be evaluated for an infection asap.
