Multiple causes: Under what circumstances this even usually happen? At rest, during activities, stress? Are you drinking coffee, energy drinks? Do you feel anxious? How long the episode last, how is trigger, how it get better? Any associated symptoms like tremors, headaches, panic feeling, anxiety, diarrhea, vision abnormalities? Do you take any medications? A doctor is like a good sherlock holmes! see your doctor.

Evaluation of PVC's: I am not sure how you know you have PVC's-I assume from a previous evaluation. If you are having  irregular heart beats and your heart rate is lower than what you are use to, you should call your doctor/cardiologist now and make arrangements for an evaluation which will probably be both an ECG and Holter monitor. I think any change in your HR from what you are use to requires an urgent evaluation.

why does my vision flicker from side to side in time with my heart beat when i would walk quickly and get slightly exerted? my bp is usually on the lower side 93/54 is this related in some way? thanks?

: It's common for one's vision to pulsate, getting rhythmically dim and bright with, or immediately after, vigorous exercise.  If this is new or there is a change in this sensation, it might be appropriate to see an eye doctor (ophthalmologist).

Blood pressure: Need to to find out the cause first; anxitey, hyperthyroidism, cardiac arrythmia can cause palpitation , hypertnesion can be secondary to the above  or unrelated , see a doctor evaluation and appropriate treatment.

when i have orgasm after masturbation i get pause in my heart beat . it pauses then race then pause and race again . it happen 2-3 consecutive . then returns to normal rhytm . is is okay things like this to happen ? i also hold my breath .

Heart rythm: Masturbation causes erection and then ejaculation. This is associated with stimulation of your parasympathetic and then sympathetic nervous system.  That would likely be causing your change in heart rate. If brief this should not be an issue. Do the symptoms come on at other times. Troubles with running or activity. and do you feel like passing out?  Those would be more concerning.
