Fungal infection: Fungal infections often develop in dark moist areas of the body such as the groin area and under the arms and breasts. He can try an over the counter anti-fungal treatment for a few days but if the rash doesn't improve, he needs to see a doctor.

Many causes possible: Without examining you it is not possible to say for sure. Vitiligo can cause what you describe along with some fungal infections (tinea versicolor). When your surrounding normal skin naturally tans, the white spots don't, so they can appear brighter. See a dermatologist to help diagnose and propose a possible treatment. Sometimes this kind of a problem can be permanent.

?spinal cord: Multiple sclerosis can involve both brain and spinal cord, and you could have involvement of the latter, even with normal spinal fluid at this point.  Other considerations should be included, as you need to get this pinned down. Suggest, an EMG to evaluate peripheral nerves, series of blood studies to assess other diagnoses, and a cervical MRI to more fully evaluate ms.
