This could be...: This could be lichen planus or lichenification of a fungal infection in the groin. It could also be psoriasis. A prescription strength topical anti-fungal and/or prescription grade topical steroid may help, but you must first see a physician for a proper diagnosis.

Could be: A simple as a basic fungal infection - so called jock itch/tinea crura.  But it also could be related to yeast, a bacteria known as corynebacteria(erythrasma), "inverse" psoriasis or just plain irritation.  It's hard to say without seeing it but odds are it's likely "jock itch".  You could consider trying over the counter Lotrimin (clotrimazole) to see if it helps.  Otherwise seek the input of your doctor.

No treatment: The post inflammatory hyperpigmentation can stay indefinitely after a long standing infection or inflammations is resolved.

Perhaps, but: The location over the upper arm, upper chest, or upper back is based on vascularity, and resultant absorption, where the medicine is better absorbed higher up.  Theoretically, it still will cross transdermally, but effect may be blunted if placed in groin area.

: Google groin rash and view images to see if it matches any. Without seeing can only suggest ringworm as most likely.  Go to uptodate.com and read more.  Then discuss with your team.

it did go away, but has reappeared.

Anti fungal cream: Lamisil has a side effect on the liver.
Apply antifungal cream- antifungal with steroid is OK.
Apply twice daily x 4 weeks
may have to be re checked again.

Body rash: Likely  pityriasis roasea which will go away on own in 6-8 weeks or eczema/psoriasis , bacterial or fungal, tinea vesicolor or related to allergy- medications ( if on any) or food -like celiac, can also be bites like scabies usually are linear pattern, rarely can be  thyroid, or liver /kidney condition, autoimmune process.  Would check CBC, thyroid , ESR. F/u w/urgent care  or doc over next 1-3 d

Yes: Need to make sure this is not Herpes Infection, which can be treated effectively and you do not have to remain uncomfortable. There are 2 different types of herpes Infection I am concerned about. 
It may not be infection at all and it can be allergic reaction to something or Dermatitis.
 All in all you need examination.

Fever/other symptom: that what they meant: symptoms that necessitate an ER visit, not related to the patch, which you want to see your primary doctor for, who would direct your care accordingly then, wish you wellness
