Minor pinch: Your dentist will place a topical anesthetic (numbing gel)  on your gums prior to the injection. You may feel a small pinch as the needle is inserted, then almost immediately, that area will start to numb. Getting a shot in the arm is much more painful.

Mouth: Yes however if the tenderness persists more than 2 days return to the dentist for evaluation of the area. This  tenderness is an uncommon occurrence.

Be careful: you most probably had a reaction to the epinephrine or it was injected into a blood vessel. In any event make sure you tell anyone working on you and they want to give you an anesthetic injection tell them what happened and insist they use a different anesthetic besides lidocaine with epinephrine. There are plenty other choices

Molecule Size: Although the below referenced article may be too technical to be of any use, in summary the smaller molecular size of the active ingredient in Septocaine may allow for better absorption into the bone for local infiltration. You may have really dense bone in the areas that needed treatment.

Lidocaine allergy?: While possible, I have yet to have a patient truly allergic to lidocaine. More often than not, patients have a reaction to the Adrenalin in the local anesthetic and say they are "allergic" to it. There are other types of local anesthetics available. See an allergist for testing. Research endodontists in your area. Perhaps a local dental school or hospital based dental residency program.

Yes: Using local anesthetic and nitrous oxide together usually increases the chances that the patient will be more relaxed for their dental appointment.

Similar: Lidocaine 5% that you are referring to is similar in class to Novacaine that most dentists use.  It is used for local numbness and anesthesia.  Heart patients, when used for local numbing procedures, can tolerate this medication without any problems.
