i've been taking allopurinol for gout for years now. two years ago they upped my mg by 100. now i am taking 200mg daily. i have noticed that my bilirubin is high. last year bilirubin was 1.2 and this year it is 2.0. should i stop allopurinol?

: High bilirubin alone is less likely a side effect of allopurinol and dose of 200 mg a day is still a below average dose if you do not have chronic kidney disease. High bilirubin can be due to Gilbert's syndrome. You can have bilirubin rechecked, not fasting. Do not stop allopurinol abruptly due to risk of acute gout attack.

NO !!: Wait until attack has subsided and slowly add Allopurinol to treatment.This drug mobilizes uric acid (so the kidneys can excrete it) and can bring on or worsen an attack.

?: During acute attacks you may not want to start it. Once acute attack resolved, it is a very effective drug to lower blood levels of utica acid, decreasing the frequency of attacks. Goal is to get level below 6.

Yes.: I rarely use Prednisone for any period for gouty arthritis attacks. Sometimes i give steroids within the inflammed joints, which  works faster, or give one shot im of steroids . I do use Colchicine almost always, but instead of using Colcrys (the only colchicne that can be sold-ridiculously priced, I use probenecid/colchicine-much cheaper and generic.  I gradually start with 100mg Allopurinol & up.

It depends: Your dosage is based on a number of variables: the medical condition being treated, the severity of your condition, how you respond to the medication, other medications you are taking, and other medical conditions you may have. Dosing can range from 100-800 mg/day. Talk to your doctor what is appropriate for you.

Yes: You should never be started on Allopurinol alone as it commonly can precitpitate gout early in the treatment. Always take Colchicine 1 tab daily with it until your serum uric acid level is stable at 6 mg/dl or less. Hold Allopurinol during the acute attack and treat the attack either with Prednisone or nsaids. Colchicine is not my first choice for acute attacks.

Possibly: Some people do well on just Allopurinol for suppressing gout attacks.  Do not stop your medications without consulting your doctor first!

Accepted Answer

Uloric: Uloric is another oral medication to use. If you have severe gout with a significantly elevated uric acid level there is a new IV medication Krystexxa (pegloticase) which has risks.Of course, diet still plays a major role in decreasing uric acid levels.
