Accepted Answer

Cycle days: Cycle days are counted from start of on period to the start of next, so if that is 34 days then around day 17 is best bet.  You might want to consider using ovulation prediction kits from about day 14 thru 19 to be more precise.

my progesterone level is 5 on day 20 of 34 day cycle.  on other cycles it wasn't more than 3. so does it mean i am going to ovulate?

Recheck: With a 34 day cycle you would ovulate on about cycle day 20.  The progesterone level increases after ovulation so you may be checking a bit too soon to see the rise.  It may be more informative to check the progesterone level on cycle day 27 to 30.  By then, if you have ovulated it will be elevated and if you have conceived it will even be much higher. Good luck.

Losing weight: To lose weight, get ur resting metabolism rate done(weight loss centers do this). This determines the total number of calories you need to lose 1-2lbs/week. Decreasing 500cal/day will give you a 1lb wt loss in 1wk; drink water (1/2 your body weight), eat small meals about 150-200cal 2-4x/(depending on your metabolism rate) and 500cal/meal at night: lean protein/veggies; avoid carbs, .

Many reasons: What you are describing may be a period.Have her do a pregnancy test and depending on result,try to consult confidentially with her doctor.Planned Parenthood might also be an option as they can help confidentially.If the pregnancy test is negative,have her discuss with her doctor other possible causes of not having a period(stress is one).Also,please be sure to use condoms every time you have sex!

Clomid (clomiphene): If your cycle is late, check a pregnancy test first before taking the provera (medroxyprogesterone).  I usually give Clomid cycle days 5-9 or 6-10, but other gyn providers use other protocols.  While twins are slightly more common on clomid, i don't adjust the dose to cause twins.  It doesn't work that way.  In addition, I do a Clomid check each cycle.  I never give an open Clomid script with refills.

Possibly: If you are unsure, do pregnancy test. This will tell you. If negative, but still in doubt and no period, repeat in 2 weeks.

Evaluation: Home ovulation tests can be useful but the more definitive way to determine if ovulation is occurring is a midluteal phase Progesterone blood test. Your Obgyn can order this for you to confirm when and if ovulation is occurring. If you back pain and cramping continue they can evaluate this for you as well.

PCOS: If ovulation is truly occurring then it is happening approximately 14 days prior to the next menstrual cycle. With a 34 day cycle, ovulation is probably occurring approximately day  20.

It happens sometimes: Many women have occasional variation of their menstrual cycles, some spotting between periods, a heavier period, a lighter period, a longer or shorter period, or even a missed period.  Having one or two of these symptoms doesn’t necessarily mean there is a medical problem, but a visit to one’s primary care or Ob-Gyn doctor is a good idea if symptoms are persistent, recurring, or worrisome.

Reasons:: Reasons for missed/late periods: low body weight, obesity, marked weight ^ or v, over-exercise, endurance athletics, breast-feeding, ^ stress, illness, eating disorders (bulimia or anorexia), anovulatory cycle (no egg released), travel, hormone irregularities, drug use, meds (i.e., birth control) or medical problems. #1 reason is pregnancy.
