Yes: Thoracic out let syndrome is caused by narrowing of the space in the chest that contains the nerves (brachial plexus) and arteries (axillary artery and veins).  Sometimes an extra rib, called the cervical rib narrows the space.  Usually it is the 1st rib and the anterior scalene muscle.  Surgery to remove the 1st rib and the ant scalene muscle can help with symptoms.

A teen using Cocaine is still at risk for heart attack too, despite their age.

Thought : Thought the location of your pain would be consistent with a heart problem, the type of pain (stabbing), its onset at rest, and your age and gender all argue against that.  I think the most likely etiology is gastroesophageal reflux (acid flowing up from your stomach into the unprotected esophagus). That's more typically burning in character, but could certainly occur when you're in bed and lying flat.  Esophageal spasm can also cause the type of pain you describe, as could other, less likely things like pancreatitis.

A family medicine doctor or internist would probably be the best person to sort through your history and do a detailed physical exam.  He or she could then direct you to the appropriate specialist, if one is needed.

While you're waiting to see a doctor, i suggest that you try some over-the counter antacids to see if you get some relief.  If the pain is accompanied by shortness of breath, sweating, or other symptoms, then you should probably go to an er rather than waiting for a scheduled appointment.

Unfortunately : Unfortunately there are no over the counter medications that treat cough and chest congestion in a 5 month old. For this reason it is important to bring the baby in for an evaluation of their symptoms. Prescription medications can be prescribed as deemed necessary. 
A few things you can do for babies this age is to put a humidifier or cool mist vaporizer where the baby sleeps, use pediatric salt drops (peds ocean spray) for nasal congestion, and tylenol (acetaminophen) as needed for fever, pain, and irritability (indicator of pain).

General doctor: See your general doctor.  You don't need a specialist for that.  The pain will go away soon.  If it recurs and it's again due to tension, you may need to discuss this with a psychiatrist - to manage stress in your life.
