6'4" tall, weigh 165, run 6 miles daily 5 days a week.  ac1 test showed 5.4.  should i be concerned about diabetes?

No: Hi. No, you shouldn't be concerned based on what you've told us. You'd be considered extremely low risk for diabetes. By the way, how are you running that much if you have CHF? It must be very mild or maybe even theoretical! Good luck!

diabetes run on b/sides of family so i like to monitor it.  when i test in am, it can read anywhere from under 100 to 105. r those reads good?

Yes: Good that you're proactive w/your health but don't get obsessive either. Best to check after fasting all night long. Small differences btwn fingerstick values &amp; blood drawn results. While yours appear fine, best to discuss w/your family doc. Consider asking for A1c test to determine average sugar level over last 3 months. Eat healthy nutrition. Exercise regularly. Keep BMI less than 25. Don't smok

Please start with a consult Psychologist. She/he will carefully work on the issues and plan things to help you stop gaining weight.
Your primary doctor can refer you to the Psychologist.

Yes, Gigantic: From population studies, at time of diagnosis, hba1c ≥6.5%, people with elevated glucose are not just starting to more rapidly develop atherosclerosis, they have been doing so for several decades, do not live as long & have more cardiovascular events than people without dm who have already had a heart attack. Plus dm have problems with a vast array of body functions, slow healing, etc.

Maybe neuropathy: If this only happens when you run it may be a circulatory problem.  It could also be a neuropathy.  The only way to tell for sure is to have your doctor have tests done for both of these conditions.

Hypoglycemia: It may be that you  are not eatng enough calories at lunch.  It is common for active people to have a low bs at end of work day.
You might try eating a larger lunch, or eating a late afternoon snack like a sandwich or piece of fruit. Best wishes.

Diabetes risks: Usually yes (due to genetic inheritance), but not always. Someone who has gained a lot of weight can develop dm2 even if nobody in the family has it. Sometimes it can happen randomly even without a lot of weight gain.

get tested: It could be acanthosis nigricans which can signify Insulin resistance. If you have a strong family history of diabetes, and you're worried about it, get a blood test to find out your levels. Ask for a test called a1c.

Reflex tearing: Your description sounds like dry eye with reflex tearing.  Try using frequent artificial tears, ointment at night, and warm compresses.
