after speaking and laughing, i have bilateral headache on my temples. i have a tmj disorder but no pain or sound or opening problems with my jaw. could tmj be the cause of these headaches ? jaw exercices don't help, how can i relieve this pain ?

: Yes, facial pain and headaches as you describe could well be secondary to TMJ dysfunction.  See a dentist, and get fitted for a nocturnal splint.  If this fails, see an oral surgeon.

obviously i need to speak w/ my pcp. i've had ^alt,ast,crp,co2, and kidney failure. i also have crohns disease, skin problems and all over pain. maybe?

Yes: You do need further evaluation and management. What medication are you taking for the crohns? Is your Crohns well controlled? All the symptoms may be related to Crohns or and medications adverse effects.  Likely may need to see GI specialist

Not related: The size of your zygoma will not cause any speech issues, as they are not involved in those functions. Perhaps having some speak counseling would be more useful. Often training and practice will help you speak clearly and better pronounce your words. Speech therapists are available to evaluate your situation, and can guide you in a program to help you.

Infection: Presence and source of infection best determined by physical exam.  Identification of specific pathogen(s) involved best determined by lab tests.  Please call your PCP now.  Infections tend to worsen rather than self-healing.  You need help--find it.

Speech : Speech may improve by simply adjusting his pd medications. There is also a specialized form of speech therapy called lsvt, proven effective. Singapore has excellent movement disorders specialists. He should consider seeing one, such as dr. Erle lim.

See details: The presence of celiac disease is confirmed by endoscopy and biopsy. A positive blood test is not sufficient.

Worrisome: Stuttering is occasionally seen in normal kids 2-5 as a transitional stage while becoming self aware while learning communication  skills. Normal stuttering usually fades within 6 mo with no external effort. When it begins later in life, is accompanied by obvious embarrassment/struggling, interferes with school work, etc.,it is time to seek the help of a speech &amp; language specialist.
