Most common:: Some medications  have an elevated risk for harming the kidneys. In the worst case, the drug causes kidney failure, while in other cases, the kidneys are damaged, but don't fail. The most common nephrotoxic drugs are nonsteroidal anti-inflammatory drugs (nsaids ) , some antibiotics, some painkillers,  radiocontrast dye and diuretic overuse.

: If you do have a kidney infection,(pyelonephritis), your doctors would certainly choose an antibiotic which would address staph aureus where sensitivity is present, to eradicate problem.  Ask the physicians how they decide what antibiotic is to be used for you.

Yes.: Many drugs can result in renal failure which will eventually lead to atrophy of the kidneys. It's rare today though since we are well aware of these drugs and your doctor should monitor renal function if you're taking them.

The kidneys may have age related small decrease in function but are not damaged.This small decrese in function generally does not interfere in the ability of the kidney to remove breakdown products of medications. Medication doses need to be adjusted by your doctor for further loss of kidney function and is also generally seen in the drug insert.

Yes: Almost all medications of any kind can affect either the liver or kidneys in large doses and/or long-term use.  Several diabetic medications do affect the kidneys, the most notable being metformin (glucophage).  That is not to say these drugs should not be used for diabetes or you cannot use them if you have kidney disease - simply that frequent monitoring of your kidney function is necessary.

High BP + Kidney Dz: Typically, medication lists for a patient with high blood pressure and kidney disease include ace inhibitors, angiotensin receptor blockers (arbs), diuretics (typically thiazide diuretics), and possibly beta blockers or calcium channel blockers (bb and CCB are never given together.)

"sore kidneys": Not sure what you mean by "sore kidneys".  If you mean having low back pain, it is more likely that the pain is not caused by the kidneys.  Most patients with acute or chronic kidney diseases do not have localized pain in the low back or pelvic area.  I recommend an evaluation by a physician that includes a full physical examination.

The most prescribed are the bisphosphonates- the ones taken by mouth are only little absorbed and are do not lead to kidney problems.  Several are  IV, with Reclast (zoledronic acid) or zoledrenic acid given usually over 30 minutes once a year.  Kidney complications can occur. Most likely if the medication is pushed in fast or if there is already  kidney disease or impaired function.  Prolia - is kidney safe.

See answer: Atrophy by definition refers to shrinking of tissue from the normal anatomical state. In that sense medications probably don't cause atrophy, but some can cause damage and loss of function. High doses of medications like nsaids and some powerful antibiotics can do this. Atrophy of the kidneys is probably more likely a result of hypertensive, ischemic (arterial narrowing) or obstructive damage.
