Unfortunately, : Unfortunately, alopecia areata can recur even years later.  I would recommend you get kenalog (triamcinolone) injections into the area again before the spots increase in size.  Other adjunctive options would include a topical steroid foam, topical immunotherapy (cream you apply nightly or several times per week) to alter the immune response responsible for the hair loss.  I often combine the steroid injections with the topical treatment and get pretty good results.  There are a few other options used less frequently that your dermatologist can discuss with you.

Depends: First you want to better categorize why you are losing hair.  The causes are far ranging from stress, nutritional, hormonal, genetic,  autoimmune/inflammatory, even related to how you chemically treat or braid your hair.  Only once you have a better sense of the underlying cause or causes can you better determine a treatment.

HAIR LOSS: Rogaine (minoxidil) seems to work well for many of my patients. A dermatologist would be one to consult. He or she might recommend an oral medication that also seems ot help.

Need evaluation.: Complete baldness (alopecia totalis) does not respond to minoxidil or finasteride, in my experience. You need a workup from your doctor or a dermatologist to identify an underlying cause, if any. Sometimes an aggressive course of oral &amp; topical steroids or cyclosporine will work, but your doctor will determine if that is a reasonable option.

Fungus OR alopecia: If the bald spots show any kind of redness (inflammation) then the likely diagnosis is tinea capitus which requires an anti-fungal presciption pill to cure (topical meds do not work) if the bald spots are just bare skin then the diagnosis could be localized 'alopecia areata'.  This condition is thought to be 'auto-immune' a dermatologist  may inject cortisone for resolution.

Alopecia vs fungus: Child probably is developing alopecia. It would be important also to rule out a fungal infection of the scalp. Best would be to see a dermatologist who can do a Woods light examination and a KOH prep and tell you the diagnosis and treatment right away. If it is Alopecia he will also be able to guide you properly and reassure you. If required further work up can also be initiated.

Bald spot: Many possible causes including simply being on his/her back too much. I would advise seeing  the baby's pediatrician first, they should be able to determine the cause and generally you should be able to see your pediatrician a lot sooner than a dermatologist. Good luck

Needs an evaluation: If the kid does have a scalp fungus which is not clear from the description,external creams/ointments are unlikely to cure it. When present, the fungus gets into the hair root, deep enough into the skin that external creams will never reach it. It may transiently imptove the outer appearance.

OTC Antidandruff : Use any over the counter anti dandruff shampoo as a face wash.  It doesn't care where you use it and works equally well all over.
