When they get biopsied, they may have some cytologic atypia, which is reported as mildly dysplastic, moderately dysplastic or severely dysplastic. This report may cause the dermatologist to re-excise the lesion to make sure that it is all gone.

100% or 0%: Don't guess odds. A suspicious mole needs to come off. It's either cancer and your life is probably saved, or it's inconsequential. A dermatologist can guess malignancy with pretty good accuracy in a suspicious mole, but it's never certain and criteria for moles that should come off are basically scientific.

Yes: Shave biopsy is not a good procedure for mole. Pathologist can only tell you what is in the tissue that was submitted, not what is in the tissue that is still part of your body. Biopsy of a mole needs to be an excisional biopsy.

Get it removed: You should see your doctor and have an examination of this suspicious lump. I should actually be removed and examined microscopically as it can be a recurrence of the same tumor, possibly it is cancerous(Melanoma). But biopsy is recommended urgently.
