It looks scaly: Scaly lesions are just that - skin conditions that appear scaly.   They are dry and usually can flake off if scratched.  Dandruff is a type of scaly lesion of the scalp. Eczema is another common scaly lesion.  Many fungal skin infections are also scaly, but some skin cancers can also be scaly, so if uncertain have a dermatologist or your primary care doc take a look at it.

Unbearable pain?: It is very unusual to have unbearable pain in a small skin lesion.  If you are putting anything on it stop.  And if the pain is truly so severe, that may signify a serious condition and if it lasts for more than a few hours, go to the ER.  Take care!
