L-spine evaluation: The least severe and easiest to treat would be piriformis syndrome. This is pinching on the nerve by a muscle in you buttock region. Given your presentation and posture that causes the pain to occur this could possibly be a buldging disc in your lower lumbar spine. you should be evaluated by your primary care provider and possibly be referred to a spine specialist if need be.

See internist: You need to be evaluated by an internist who can then direct you to the correct specialist (rheumatologist, orthopedist etc) .  The unintentional wgt loss is of concern although given your physical suffering, depression is top on the list of probability.  Don't delay further in seeking medical attention.

The treatment varies depending on the cause.

No POTS and: deconditioning are NOT the same....the former is a disorder of the nervous system which contracts the peripheral vessels when upright position is assumed thus causing a drop in blood pressure and a corresponding increase in heart rate! A rapid heart rate is however sometimes due to deconditioning and slow careful exercise will help! Exerciseis USEFUL in pattients with POTS but not "curative"!

The pt feel light headed sweaty nausated and can even pass out. Medicine to keep BP up as well as salt retaining steroids help.Support stockings and staying hydrated also help.
