?Black: Hopefully it is a dark blueish color from a bruise, as black may signify an ischemic condition, which would be serious.  Get the area looked at by a dr. to be safe and get properly diagnosed and treated,  best wishes to you.

Tophaceous Gout: Gout can occur in just about any soft tissue in the body. The accumulation of uric acid crystals in the soft tissue forms nodules or masses called tophi. The singular of tophi is tophus or stone. These can become quite large and disfiguring. They can rupture through the skin and destroy not only soft tissue but joints. Tophi formation is the result of chronic high levels of uric acid in the blood.

Gout: Gout can present as intense pain in any joint of the body.  Having said that, the gout crystals tend to migrate to the coldest parts of the body which is why we normally see it in the foot.  Signs of a gouty attack would be swelling, redness and pain in a joint.  Gout normally affects 1 joint at a time.  I would advise evaluation by a podiatrist since many things can cause foot pain.

Get him treated: Gout is caused by increase blood levels of uric acid and can be treated suscessfully with long term medications.
During acute attack you need to use anti inflammatory NSAID drugs like indocin, (indomethacin) Motrin or naprosyn
but long term you need meds to bring your uric acid down and need to go on a special diet to avoid too much meat and beer atc, 
he can also use allupurinol
so check with your doctor/rhematol.

Podagra: you likely have an attack of Podagra. Best will be either steroids or Colchicine, depending on your comorbidities and response to previous treatment. Your uric acid will have to be addressed with maintenance therapy later.

MRI: Depending on the type of foot surgery, it should feel better by now. An MRI might give the answer. I doubt gout, but you could have that tested for.

The : The longer a patient has gout, the disease typically will involve more joints.  It generally startes in the lower extremities (feet, ankles, knees), but can involve the upper extremities (hands, wrists, elbows).  Shoulder symptoms are less common, but can be related to gout in some cases.  I would recommend a thorough rheumatologic evaluation.  If  your symptoms are all related to gout, you may need to be on long-term gout medications such as allopurinol.  On the other hand, the symptoms could be related to another type of arthritis (and possible muscle/tendon problems in the shoulder) and should be evaluated by an expert to make this distinction.  Hope this is helpful.
