Nonspecific: A bone scan finds areas where the bone is turning over faster, usually repairing an area.
The base of the big toe can be an area of arthritis or trauma. Another possibility would be low grade gout or perhaps other arthritis.
Correlation with clinical history is needed. If the area doesn't bother you, the finding may not be important.

Hypothyroid symptoms: Common symptoms of low thyroid = hypothyroidism are:  fatigue, feeling cold, constipation, weight gain.  It can easily be diagnosed from the blood test checking T4 level as well as TSH level .  Consult your doctor for your symptoms and have he/she check the thyroid level for you

UPJ obstruction: Relieving the obstruction on the left side, and improved function, may take the pressure of the right kidney and may not be a problem. It needs to be followed closely with repeated scans and renal u/s.

Increased activity: Strong uptake on bone scan means increased metabolic bone activity.  This could indicate inflammation, fracture, infection, and/or bone tumor.  These results usually are considered in the face of plain x-rays and exam to determine the diagnosis.  Sometimes mri/ct scan are also needed.

It does not make: sense.  Suspect laboratory error for the second TSH but there patients with subacute thyroiditis who show unusual test patterns.  Uptake is ususwally very low, though.  Your doctor will go by symptoms

Thyroid?: I presume thyroid is what you refer to. It would be highly unusual for an endocrinologist to not be able to distinguish between hyperthyroid and hypothyroid.

No.: We do this all the time. If symptoms bother you Cytomel (liothyronine) may be given for up to two weeks before the exam.

Depends on insurance: At our hospital, a thyroid uptake and scan cost just over $1000. The cost to the patient is different and depends on your insurance.

Accepted Answer

Possible hypothyroid: Thyroid uptake depends on thyroid iodine trapping which is proportional to the rate at which the thyroid is making hormone. The total amount of iodine in the diet is also a major factor. If you use iodized salt and live by the seashore  your body will have more iodine available to dilute the test dose; thus giving a lower uptake value.High sensitivity TSH is a much more reliable indicator.
