Conundrum : You have a constellation of symptoms which makes diagnosis challenging.  You need thorough evaluation with a physician & potentially a consultant or two to help solve (& treat) this mystery.

It can, but...: ...Its far more important that your cultures or sputum smears clear of the organism. After one month, if your cxr is not better, we might not do anything different, but if your cultures stay positive, we would probably change your medications.   The cehst x-ray improvement will lag behind the more important microbiologic response.

Could be ....: Many things.  Some of the most common causes of this include a medication side effect, reflux (can be silent reflux) or cough variant asthma.

Discuss with his doc: He may not need tests but your physician will want to ask a few more questions to decide if there is a concern. Rr 40 is fast  even with a fever if he is at rest and fever for 13 days is concerning. Good chest xray does not prove anything.

Pre op: We do preop ECG and chest xray to see that no cardiac or pulmonary surprises are in store for the patient and surgical team.

what is the difference between atelectasis and pneumonia on a cxr?

Ask the doctor: They can look similar and sometimes it's difficult to know for sure by chest xray whether it's atelectasis (partly collapsed lung) or pneumonia. Often a CT scan is done, which usually can make the distinction. Treatment often is the same, so it may not matter. Discuss with the doctor(s) treating the patient.

Was a marker used: I have been confused by similar findings and have found an external metallic marker was used. If not get the radiologist to further explain it.

It may be GERD: One of the common cause of waking up at night with complain of can not breath is gastroesophageal reflux dut to regurgitation of acid, it may or may not be the cause but it is worth ruled outmay be your doctor will recommend a trial of acid supressant therapy
discuss with your doctor
it can also be sleep apnea and she may need sleep studies to rule that out.

Emphysema: While no test is 100%, assume the test was administered in the 'right' way because 95% of the time, repeating it gives the same results. Hyperaeration (too much air in lungs) can be artifactual but the Radiologist (a good one, anyway) would note that by careful observation (lungs vs. bones). The signs &amp; findings point to obstructive lung disease (COPD/emphysema). TTYD or discuss on HealthTap Prime
