Side effect: Many narcotics have the common side effect of itchiness related to a histamine release in the skin, thus there is itchiness. However, this symptom alone is not necessarily allergy to the rx, just adverse effect.

Depends: There is a difference between addiction and physical dependence. Daily opiate use can lead to tolerance which you describe, and withdrawal if discontinued. Addiction is characterized by a group of behaviors, one of which is continued use despite adverse consequences. If you want to discontinue the codeine, the best way is tapering the dose over several weeks.

Maybe: Tramadol is a centrally acting opioid used for moderate to severe pain. It also is a serotonin releaser , and norepinephrine inhibitor; actions which most other opiods do not possess. Codeine is metabolized to morphine, but since up to 7% of the population may poorly metabolize this drug, you will not obtain pain relief. Codeine also results in more nausea, itching, and rash compared to tramadol.

Both: Yes CODEINE can cause hallucinations and definitely can produce bad headaches in people who are intolerant to it...although in many instances it can be used to treat ACUTE headaches.  I never use it for that purpose because of its side effect profile (mostly GI upset) and plus there are better agents for headaches and pain in general than codeine nowadays.

Migraine: Really should see your primary care doctor or a neurologist if you have migraine headaches. Sometimes there are much better controller medications or treatments to help for the migraine. And it might not even be a migraine. It also depends on your allergy to codeine. If you just get itchy, take a non-sedating antihistamine to decrease the itch but the Codeine can help the headache.

No: For severe cough, we may prescribe Robitussin (guaifenesin) w/ codeine. It will help w/ cough suppression, illness pain, and insomnia. For people who do not tolerate codeine, vicotussin can be used; but that has hydro-codone, which you say you're sensitive to. That pretty much rules out the narcotics for cough suppression. You could use tessalon for great cough control, but without the sedation nor pain control.

Dextromethorphan: Dextromethorphan is present in many otc cough preparations, such as robitussin, (guaifenesin) delsym a& bentylin dm to name a few.  If your cough persists longer than 3 weeks, see your physician.
