Shoulder: I would suggest an x-ray to rule out a fracture after an adequate exam to evaluate the soft tissues around the joint. If the x-ray is negative and pain continues with conservative therapy, then an MRI may be needed to rule out a rotator cuff tear.

Could have cracked  : I hope you did not crack one of your ribs. Usually a broken rib causes much pain on taking a deep breath or if you try to cough. You should see your doctor to confirm it one way ot the other..May need to take an xray to check it out. Also take pain medicine to relieve your pain. But a vsit with your doctor may be most useful.

Not angina: "texidor's twinge", "stitch" or what-have-you. Your description is classic. This is very common and i believe it's caused by a little transient wrinkle in the pleura. It vanishes when you fill your lungs to maximum capacity, right?

Try analgesic balm: If this is a new problem as a result of trauma ,you could try an ice pack  placed on this area every couple of hours.As well as over the counter pain meds.Over the counter analgesic balm may help.If these fail see a doctor.

See your doc: You should have a cardiac cause ruled out with an cardiac work up. Other considerations include a thoracic outlet syndrome. Please see your physician for an evaluation and further guidance.

Get to a doctor: That is a significant combination of symptoms. It could be asthma or pneumonia or worse. A good doctor will be able to tell the difference. Get to one asap.

See below:: See an orthopod for the arm/shoulder pain and your gp for the chest pain, difficulty in breathing. The arm sounds more like cubital t syndrome which is known to move towards the shoulder or chest/ upper back. It does not happen with tennis elbow. You could have both though.
Do you have RA as you are on enbrel (etanercept)? Why are you taking inh (? Tb)? So maybe a chest physician referral maybe helpful.

Pleurisy? : The breathing pain suggests of the following 3: pleurisy, pericarditis, or muscle strain. Do you have fever, shortness of breath, or upset stomach/diarrhea (suggesting viral illness)?  If persisting, you should probably get checked in an emergency room.
