Breasts engorged: Breasts are swollen and firm due to breast milk excess. This often happens when milk production exceeds amount of milk baby is taking.  Engorgement also occurs when "milk comes in" switching from colostrum to mature milk several days after birth.

breasts engorged and hard after miscarriage at 4 weeks. with my 2 kids i had a big milk supply, but this was a very brief pregnancy, only a little liquid is coming out, and it is painful. i am massaging and trying to express, but it is not working?

: It is certainly unusual to get breast engorgement after such an early miscarriage, but massaging and trying to express can only perpetuate the problem. Avoid stimulating the breasts further. Wear a well fitting bra, restrict fluids, and apply cold packs. It should start to improve after a day or two. See a doctor if you develop a fever or red areas.

Mastitis or Mondor's: This could be due to a variety of causes which i would discuss with your lactation consultant it gynecologist. Antibiotics and warm compresses may help to alleviate infection or congestion.

Yes: Clogged milk ducts can cause painful hard areas on the breast.  To relieve them, try warm compresses, anti-inflammatory medication like Ibuprofen or Naproxen and gentle massage. Continue to feed the baby.  Some lactation nurses recommend the supplement, lecithin, three times a day to improve milk flow through clogged ducts.

: You have to miss your period first and if yes, pregnancy test will give you the answer. 
The breast symptoms will not come early during pregnancy.
If you are not pregnant but you have bilateral nipple discharge, you need to see a doctor.

Few days: If you are breast feeding and you have experienced lumpy tender breast/breasts as you are learning to breast feed, you may have mild engorgement. If you have pain and fever call your OB about need for medication.  If no fever, try pumping breasts for 3-5 minutes before breast feeding to get milk flowing and unclog ducts. Usually 2 or 3 days of this before feeds will resolve problem.

Breastfeeding?: It sounds as if you are breastfeeding? If so a clogged duct can present as a tender red area of the breast.  The itchiness also concerns me for a yeast infection, especially if the nipple is involved. Try a warm compress on any red area tonight, be sure the baby is draining the area by compressing the milk ducts in that area.  See your physician if this is not better in 24 hours, may need yeast rx

Lump in breast: A persistent lump in your breast should be evaluated by your doctor.  If painful and red, it could be a symptom of an infection which may require prescription medications.

Hormone dysfunction?: Other than an infection, trauma, or rare tumor, the only reasons for breast engorgement are hormonally related. This could come from your menstrual cycle variations or early pregnancy. An evaluation by your gynecologist should answer your concerns, and provide relief through proper treatment.

About 2 to 4 weeks: You can shorten that time by using a good breast binder which will suppress milk production sooner.
