All these changes can develop quite quickly( in 4-6 weeks) in case sof  acute leukemia although chronic leukemias take several months to evlove enough to affect the peripheral blood.

Mostly no.: If by worms you mean, intestinal worms, these usually do not cause blood work problems; some worms may cause increase in eosinophil type of blood cells, and others may be associated with increased ige levels. If you have been tried on antibiotics and still there, even if the blood test (which does not always show all inflammations or diseases) are normal, i would suggest a biopsy (not needle) to rule out other things like malignancy or weird infections like mycobacterial. Get advise from your doctor.

Ecg conditions can cause blood clots also-  low ejection fraction, and atrial fibrillation would lead this list.
