Lymph nodes "taste" the lymph for anything bad  like bacteria/viruses/cancers, etc. When it detects a problem, it alerts the immune system and your body reacts.  They enlarge in response to the inflammation they create when they react.

: If you recently had symptoms of a viral infection you could have those nodes which are caused reactive lymph nodes. They may occur without any associated symptoms as well. The condition is benign most of the time.
However, if the nodes are enlarging and persistent after a couple weeks, you should have them checked.

Lots of things: The lymph nodes process body serum to remove debris &amp; germs. The swell when working. Something as simple as a vaccine in that leg or a local skin rash can trigger the swelling. Once they swell they seldom go down to resting size in less than 3 wks. Call if you wish.

Infection: A lymph node that is swollen and tender is typically a sign of infection.  Often, if it's is a single lymph node that is swollen and tender, there is a bacterial infection in the region drained by that lymph node.  Bacterial infections are best treated with an appropriate antibiotic in most cases.

Yes it is a cancer: Adenocarcinoma is one type of common Cancer which can arise from any gland forming organ such as Breast, Colon and lung. If left untreated for a long time, cancer cells spread and can involve regional lymph nodes and later spread to the internal organs. I hope you are under the care of a cancer specialist(Oncologist) who can guide you further and offer the required treatments.

i've had a bilateral neck dissection for thyroid cancer in 2011. my neck has been swollen for 3 years. should i see a lymph node specialist?

Follow up: If you had thyroid cancer  at the moment it would be a good idea if you regularly touch bases with an oncologist. Not sure what advise you were given after the surgery but if you have new symptoms it is definitely time to pay them a visit.
