Fewer echoes on US: Hypoechoic describes the appearance of a nodule on ultrasound (us).  It refers to a nodule that reflects back fewer sound waves compared to the normal thyroid tissue around it.  While most thyroid cancers are hypoechoic, most hypoechoic nodules are not cancers.  But a hypoechoic nodule may have a slightly higher risk of being a cancer than other nodules.

i have 2 thyroid nodules and 1 is hypoechoic and the other complex should i worry?

Depends: Hypoechoic, hypervascular, or calcifications on ultrasound need to be addressed particularly if the nodule is &gt; 10mm. The solid component of a complex nodule can be benign or cancer. Typically a nodule greater than 10mm should be biopsied by ultrasound or smaller if there are calcifications or prior history of radiation or family hist. There is no ultrasound pattern that is diagnostic for cancer.

good morning. last month they made me an us and reported that they found hypervasular thyroid nodule (0.9 x 0.8mm) and the nodule is hypoechoic. is it something i should worry about?

: Thyroid nodules are not uncommon.  Not all nodules are dangerous, or cancerous.  This may be a little on the small side for a biopsy which often is the only way to know for sure.  Speak with your doctor and ask their opinion.  I personally would not be that worried about one small nodule under 1cm in size.  They may also suggests other imaging such as with nuclear medicine imaging.

Reasonable: A 7 mm thyroid nodule is considered well under the 1 cm size at which we start to be more concerned. There is no reason to aggressively pursue earlier follow up at your age.

Needs FNA: Most nodules of the thyroid are benign.  However, fine needle aspiration (FNA) biopsy of a nodule is indicated in your age group for nodules over 1 cm in size.  Having increased vascularity raises the index of suspicion slightly.  You should have thyroid function (free T4, TSH) checked prior to biopsy.

Uncertain: There are various characteristics of thyroid nodules on ultrasound, including hyperechoic, hypoechoic, isoechoic, cystic, complex, vascular, calcified, microcalcifications.  Unfortunately, none of them can tell you for sure whether the nodule is benign or malignant.  In your age group, we would be recommending biopsy of the nodule if it is 1 cm in size or larger.

Inflammation : A hypoechoic nodule could compress normal tissue and vessels .  Inflammation around nodule might also increase flow.

in todays us hypervascular thyroid nodule 9x8mm which is hypoechoic and internal vascularity remained the same as 2 months ago. they told me that they keep checking it for 2 years. it can be something during this time? after 2 years it will be safe?

: They do serial ultrasounds when the nodule is this small to see if it begins growing. If it begins growing, they can then biopsy it. But, if it never grows, after 2 years, by definition, it would be benign (malignant tumors begin growing in that time). Typically, they do the ultrasounds fairly frequently and then begin spacing them out a bit if there is no growth. Hopefully, it will prove benign!

No.: Most thyroid nodules are benign.  However, there are various imaging/clinical features that help determine the probability that a particular nodule might be malignant, and therefore require a biopsy for definitive diagnosis, so more information is needed to make an assessment.  Talk to your doc about the results.
