Usually nothing: Ovarian cysts are normal; we only become concerned if you have too few, too many, or if they are large and/or solid depending on the age.  Cysts need to rupture each month in order to release an egg (ovulation).  Depending of the quality of the cyst, you may experience bleeding and or pain.  Sometimes, you will not notice it unless an ultrasound reveals suggestive findings.

Sometimes: Some women are more sensitive to that pain, and experience a pain in the mid-point of their menstrual cycle when an egg is released. Usually if they do note something it is mild. Occasionally, a cyst can rupture through a more vascularized area and cause internal bleeding, and this does result in severe pain.

Ovarian cyst: When an ovarian cyst ruptures, some blood is released into the pelvis which will cause pelvic pain.  Most of the time only pain medications are needed, since the majority of cysts are self-limited.  However in rare cases, the ruptured cyst can continue to bleed, and be life-threatening.  Imaging (ultrasound) in an er  and lab work are helpful.  And rule out more serious causes such as ectopic preg.

Sometimes: If the cyst produces high levels of female hormones, when it ruptures or goes away, you may get a period, sometimes heavy. Oftentime, you will get no bleeding at all from a ruptured cyst.

Some times there may be a slight discomfort, and rarely there can be bleeding when the egg is released.  The small amount of fluid released at ovuation is quickly absorbed by your body.

Generally No: If an ovarian cyst is of moderate size and ruptures, you may have some short-lived pain, but it should not harm any remaining egg cells in your ovary.
