Pos HSV2, neg HSV1: The lab report should have notations about this but maybe hard to understand (written for HCPs, not patients). With the usual HSV antibody tests, over 3.5 is definitely positive, and under 0.9 is negative. (Between those numbers, it's sometimes hard to know.) Almost certainly you are infected with HSV2, i.e. genital herpes, but probably not HSV1, which mostly causes oral herpes. See an STD expert.

: In depends on the cut-off value for a positive result. If these numbers are only slightly elevated, you might not be infected despite the positive results, especially for HSV2. (Herpes blood tests often are unreliable, which is why most experts recommend against testing if there are no symptoms or other especially high suspicion of herpes.)

what does a hsv 2 index value of 5.64 mean? i was reading an oxford hsv2 study and they indicated that scores 1.1-3.5  were considered low positive and scores >3.5 are considered to be high positive. i received lab work of a value of 5.64.

Previous infection,: testing can show if an individual has ever contracted an HSV infection. It is much harder to say the actual time of onset, unless active lesions are seen for the first or recurrent times.

for an igg test for hsv2, does an index of .90 mean i have it but the virus is dormant or inactive? my doc said .90 means i have it but since the value is so low, im trying to understand if its inactive (i have no symptoms) or im negative?

: An index value of 0.90 is technically borderline; the test is not officially positive except at index value 1.1 or higher. Even then, a positive result is not reliable except for index above 3.5. At 0.9, it is very unlikely you actually have any HSV2 IgG antibody and probably are not infected. If recently infected, your test might be becoming positive. Retest in a few weeks to be sure. how likely is it to pass on anything, if i came back with hsv2 equivocal results after testing neg 4 months prior (1 new partner between test), if i want to be intimate with a new female partner, would she be at risk of anything? what if i take meds?

: HSV is extremely contagious, and once contracted is possible of transmission even without visible ulcerative lesions. Treatment can hasten resolution of active lesions (antivirals like acyclovir, Valtrex are very effective), however the virus exists inside the nucleus of cells, and cannot be eliminated completely.

i tested + for hsv2 3 days after unprotected sex with my partner who tested - 2 days after i tested. is that an accurate testing for my partner?

Herpes testing: Herpes testing is more challenging than other STDs. A  swab can be done if lesions are present to directly look for the virus. Otherwise blood antibody testing can be done to evaluate for past exposure. It takes about 6 weeks after exposure  for antibodies to show up in blood. Swab testing can only be done when lesions are present

HSV testing: Both are good. A PCR blood test looks for circulating viral DNA. The IgG look for the development of an antibody against the virus. If these are not present 7 weeks after exposure then the most likely conclusion is that you did not acquire it.

i have a strange phobia i had hiv 4th gen and hepatitis testing with all other std tests negative at 6 months except hsv2 i have not tested for herpes but i still fear that all the other tests have missed infections?

That means...: That means the patient should talk with her ordering physician and review the lab test result to see why it says "positive equivocal." Usually, there are only 3 possible results on HSV2 tests (either antibody, nucleic acid, or culture tests). The 3 possible test results are:  Positive, Negative, Equivocal. The result will only be one of those 3, not two of those 3.
