Many possibilities: Her doctor is the best person to figure this out. Problems in the joints, nerves, or blood vessels may be causing these symptoms. A primary doctor or rheumatologist may be helpful. Rheumatologists diagnose and treat joint and musculoskeletal conditions.

Circulation.: I am assuming you mean swelling from excess blood flow and not actual blood.
Could be a DVT in the arm. It would be important to have an exam done and maybe some testing.

Need medical exam: Many people fear a heart attack if pain radiates down arm, but that is extremely unlikely in a 19-year-old woman. More likely is pinched nerve in the neck, but impossible to know without a medical exam.

Swollen vein: For the vein to be swollen, it has to be pressed upon by another structure or contain a blood clot.. Since you're also experiencing discomfort at the fold of the arm, I'm concerned about phlebitis of that vein. Has the vein been traumatized in any way? I recommend consulting your physician or a vascular surgeon for evaluation,diagnosis and advice.

Did : Did you get an mri?  I think an MRI would be helpful to see if there is any sort of mechanical block to your extension such as a loose body (a piece of cartilage that is stuck in the joint).   If your last three fingers are getting numb, this may be sign of an ulnar nerve compression.
