Blighted ovum.: That would depend on how advanced the pregnancy is.  By 5 weeks there should be at least a yolk sac present in the gestational sac in the uterus. By six weeks a fetal pole should be seen and 85 to 90% of the time a fetal heart beat is noted by six weeks.  If there is no fetal pole or yolk sac present by six weeks, it is unlikely that the pregnancy is normal. That is referred to as a blighted ovum.

It depends: how far along you are.  Early in pregnancy, at 4-5 weeks, it is normal to see just a sac because the embryo is too small. After 5-6 weeeks, it would be abnormal to see only a sac.

i went in for an early ultrasound. didn't see anything, no sac, nothing. beta hcg was 528, it normal to see nothing at those levels? i go back in week

4-6 weeks: The implantation of the blastocyst into the endometrium occurs at approximately 3 weeks and 2 days after conception. A typical ultrasound machine using present-day technology allows for visualization of the gestational sac as early as 4-6 weeks. 

Keep in mind, this early 4 week figure is for trans-vaginal ultrasounds.

Talk to your doctor: most women have small amount of fluid in the cul-de-sac and you don't need to worry about it. The anterior cul-de-sac is the space between your bladder and your uterus, the posterior cul-de-sac is the space between your uterus and your rectum. Talk to the doctor who orders the test for more information.

Ultrasound: Yes, depending on the type and sensitivity of the machine, you may only see a sac without the fetal pole inside.  Things will be significantly different in 1 week.

had my first ultrasound and my sac was empty didn't see no baby but the sac measured that i was 11 weeks .. what could be the reason for that ?

Unfortunately,: it could mean a failed pregnancy. The fetus should be visualized at 11 weeks. Please talk to your doctor about it.

Pretty standard.: Although there is usually some plus/minus variance in the measurement, these measurements have been established and standardized so they are pretty accurate.

my wife had first ultrasound at 6 weeks. the doctor was able to see a sac, but no heartbeat. should she worry?

No: Won't often see a heartbeat that early. Check again at 10-12 weeks.
