Truly increased echogenicity implies renal disease but is very nonspecific, there are a number of entities that could manifest that way.  The finding must be interpreted in the context of your clinical picture to determine the significance, if any.

: Regardless of your eGFR you are a patient with chronic kidney disease. If possible, the current kidney stone, even small, should be eliminated. You need to follow closely with a urologist or nephrologist with high expertise in stone prevention and treatment or a team of both specialists. Kidney disease will progress but at a slow pace with effective measures for stone recurrence &amp; pyelonephritis.

It's Subjective: Generally, increased echogenecity indicates fatty infiltration, and coarsened echo texture (of the liver) could indicate cirrhosis (or hepatitis; among others), however, the findings are very subjective, cannot be graded and vary from observer to observer. If you have known liver disease (or even if unknown), consult a gastroenterologist for possible liver biopsy.

Tough Question: Boy thats a tough question. Usually things that are bright on ultrasound are calculi (stones), air, or fat. If it were air the pt would likely be very sick. Fat is almost always benign, and stones can cause symptoms ranging from mild pain to sepsis.
