The uterus is generally harder to visualize in its complete extent on transabdominal images, but this is a close approximation of what is seen in transabdominal ultrasound (transvaginal is the better of the two).

Pap smears: Pap smears allow  pathologists to evaluate cells from the cervix (the outside or ectocervix and inside or endocervix).  Most of the time we do not find cells from the endometrial cavity  in the test slide because the pap smear collection brush should not go into the endometrium.  Additional biopsies are required to evaluate endometrial pathology.

A thought: There are cervical cells from the cervix.
There are endometrial cells which are the uterine lining.
The goal is to detect cervical or uterine cancer and the best hope is for normal cells!

See below: Endometrial cells are what make up the uterine lining, so this is a normal comment.  If on the other hand, you are having abnormal bleeding (as a reason for the ultrasound that you had performed) and there are endometrial cells on a pap smear, then you definitely need further evaluation of the uterine lining to rule out pre-cancerous changes called hyperplasia.

Tested for gene?: Have you been tested for the BRCA gene yet? Breast, ovarian and colon cancer are associated with that gene. Uterine cancer usually will show a thickness in the lining of the uterus (endometrium) on ultrasound or irregular bleeding. Keep seeing your MD for regular evaluations and good luck.

Fibroid cramping: The size of the fibroids are immaterial if you are experiencing symptoms like cramping, bleeding or pain. In those cases your MD should discuss with you if the symptoms interfere with your activities of daily living or not to consider a therapeutic approach to the problem or not.

Normal: It is normal size If you have already been pregnant in the past the uterus might be slightly larger and still normal.
