Chest pain: Chest pain is the typical symptom of heart attack. Other symptoms include dizziness and lightheadedness, and sometime nausea and abdominal pain, although hard to distinguish from symptoms of jogging. If they are occurring at levels of exercise lower than you usually tolerate, think about it. But chest pain is not a sign of exercise and should be addresses.

Less damage: Mild means that only a small part of the heart muscle is lost so that the effect on a patient's performance is minimal. This is still dangerous however; arrhythmias that can be lethal can occur in any size heart attack. Now that we have cardiac alerts at hospitals early intervention is reducing the amount of heart damage that occurs with most heart attacks.

The blood tests are transient.
Cardiac cath and lv gram can also invasively show.

Go to a doctor: He would need. Comprehensive cardiac risk evaluation and aggressive medical treatment to avoid a repeat event.  A heart healthy diet, participation in cardiac rehab would also be important.  If he smokes has to quit.  If heavy needs to lose weight.  There are standard medical treatments for this scenario and ideally he should have been evaluated and treated at the time of hs original event.

No, as long as: You manage your risk factors and adapt your expectations. Work closely with your doctor. This is a wake-up call to radically improve your lifestyle behaviors. 
If you have not already do so, you may want to look at the work of dean ornish in using lifestyle changes to reverse heart disease.  (google his name). It may not suit you but inform your cardiologist if you decide to adopt his approach.

Ankles: Since you already said you had a mi the ankles swelling could be from the medications you are on. Many have side effect of swelling. Would also have circulation to legs checked.

Diabetes control: Is his blood sugar unstable? With his medical conditions his physician should be consulted to ensure that any significant depression is diagnosed and properly treated.

No and Yes: Well... I guess kinda :) and yes people have survived heart attacks without going to the hospital. Although, I obviously wouldn't recommend it! ;)

Angina: Angina is chest pain that occurs when there is a partial blockage of one of the coronary arteries. This is as opposed to heart attack when there is a near total occlusion.
