Fludrocortisone: Be sure to increase enough salt in your diet and drink adequate amounts of water. Increase in dose beyond 0.3 mg per day may not be effective but in very severe cases of postural hypotension  cautiously increasing dose by 0.1 mg per week until a dose of 1.0 mg per day has been prescribed. This is a very high dose. I wouldn't recommend this without your physicians ok and supervision.

Several things: First, one should undergo several basic tests, including a careful recording of ones history, check of ones blood pressure sitting and standing, blood tests to look for identifiable medical causes of the syndrome and a search for commonly associated medical conditions.  Treatment can begin with a mild tilt of ones bed to keep the head of the bed 6 inches higher than the foot; increase oral fluids.

Yes: BP drops the heart rate accelerates to compensate. Cardiac out put is a function of stroke volume and heart rate. The blood stays in your feet so that there is a relative decrease in stroke volume and the heart rate goes up (tachy) and tries to maitain perfusion to vital organs such as the brain. If it is successful, you may feel dizzy and then recover and heart rate drops. If not pts faints.

Physical therapy exercise can help retrain these nerves, often much more effectively than medications.
