No distractions: It may be from the fact that it is quiet at night and that there is nothing else for the brain to focus on. It could be that her blood pressure is higher lying down than sitting up. The carotid artery sits close to the eardrum, so the force of blood through it may cause a sense of a "heard heartbeat".

For sure!: Your heart responds to Adrenalin and a scary dream can pump a lot  of it into your circulation. In the most extreme of circumstances, it's actually possible to be "scared to death" - this is the mechanism of so-called "voodoo death" in which a pre-scientific person is so scared of black magic directed against them, that they have a serious arrhythmia and die. Nightmares are usually harmless though.

Yes: Your HR and your BP continually fluctuates throughout the day (and night). As you go about your life doing whatever it is you do you respond to it both physiologically and psychologically and these in turn call upon our heart to pump more or less blood. And so it goes...
