Sinus cyst: Cysts or inflammation of a maxillary sinus can cause cheek discomfort and headaches but double vision would not be usual. An otolaryngologist(ear,nose and throat specialist), opthalmologist or neurologist could advise you further.

Not likely.: Your symptom complex  encompasses neurological, cardiovascular, and potentially vestibular systems all of which may be connected.  However, they cannot be attributable to a 6mm pineal gland.  A thorough history and exam in person are necessary to have a chance at a diagnosis.  It sounds like someone has already started  the process so I suggest you continue.

i had double vision, was sent by doctor to vascular surgeon, had temporal artery biopsy that was negative. i also saw rhematologist who stopped prednisone. what could cause pain at incision and in temples on both sides with headache after surgery?

: Incisional pain usually subsides with time unless a painful nerve swelling (neuroma ) develops which is very rare. 
Headaches have many potential causes and are best evaluated by a headache neurologist.

Double vision: New onset double vision should be evaluated immediately, as there are a large number of neurological and vascular causes, some of them life threatening. With headache present as well, this would be an appropriate use of an er. They would be able to do all necessary testing and have all specialist referrals available to make a diagnosis.

Go to ED: Those symptoms could indicate a serious problem - you need to be evaluated promptly. You should go see your doctor now, if that is not possible then go to the emergency department.

Migraine ?: Migraine headaches can cause a sudden but temporary strabismus (eye misalignment).
