Type II Diabetes: is from lifestyle choices, i.e., poor diet w/lots of junk food, lack of exercise, etc.  The body makes insulin but it doesn't get into the cells and we cannon make ATP for energy.  Type I Diabetes is when the pancreas does not make insulin and is typically diagnosed early on in life.

Multifactorial: No one knows exactly what causes type 2 dm, but obesity seems to trigger this in people who are genetically susceptible.  Some are inherently Insulin resistant. Other associated conditions increase risk, for example pcos, hypertension, sleep apnea, gestational diabetes, etc. When blood sugar rises quickly or not, it really depends on how high it goes and how the body does or doesn't respond.
