Conngestion: When you insert the spray make sure that you not push the tip against the side wall of the nose.Also keep your head horzontal. When used properly th action of your finger on the trigger should cause a " swoosh " as the medicine is delivered in a fine spray.You inhale at the same time and if it drips out of your nose you have not been firm enough when you " pulled the trigger"

: While Fordyce spots on the penis can become irritated during sex,I'm unsure if this also applies to those on the labia. A dermatologist is the specialist who deals with treating symptomatic ones. Don't pop or scrape them as that doesn't get rid of them and they may become infected.  Abstain from intercourse while they're symptomatic.

??: Can't answer this better than the experts who have already seen you.  Need to listen to them.   I am sure that biopsies have been done and that we at least have some idea about what is not causing your symptoms.  By this point we also likely have an idea about which treatments work and which don't.
