Same as in children: If you truly have a middle ear infection, otitis media, and the cause can be proven to be bacterial, the same bacteria that infect the middle ears in childhood, cause otitis media in adults.  And therefore, the same treatment is administered.  So, first, an appropriate diagnosis needs to be made and if the cause is bacterial, the correct antibiotic needs to be given for 7-10 days.

OM symptoms: can be variable.  Some symptoms include fullness, hearing loss, pain, ear drainage, dizziness, and fever.  Treatment of ear infections is individualized, but usually consists of some form of antibiotic.  Secondary medications may include  decongestants, mucolytics, antiinflammatories/steroids and pain medications.

Pain relief: An infant or child can benefit from Ibuprofen in proper doses every 6-8 hours. Letting them snuggle with a heating pad at low heat can sometimes diffuse the pain .Persistence in discomfort after 3 days is worth an evaluation for purulent otitis which requires abx for proper healing. Untreated otitis may self  heal or go on to chronic mastoid disease & hearing loss.

Route: Ototoxicty is usually associated with systemic antibiotic and/or chemotherapy. Drugs which are taken by mouth or IV and go throughout the body. Certain chemotherapy agents are more likely to cause ototoxicty. Some antibiotics can cause ototoxicity as well (gentamycin/vancomycin). Drugs to treat ear infections given by mouth rarely would cause ototoxicity.
