i am having surgery... the me wants to put my on ir nucynta (tapentadol) instead of vicodin. how would it compare to vicodin? what would be an equivalent dose

About equal: The dosing of IR Nucynta (tapentadol) and hydrocodone (the narcotic in Vicodin) are pretty equivalent.  No table I could find shows direct comparison of Vicodin to Nucynta (tapentadol). But 4mg morphine=12mg Hydrocodone. 4mg morphine=10mg Nucynta (tapentadol). So pretty comparable. Hope this helps. Best of luck. Dr. R

Similar: Nucynta (tapentadol) is similar in strength to vicodin and other opioids that are given for post op pain.  There is no equivalent dose, so there is no answer to that question.  When it was studied it was compared to oxycodone and was found to be just as effective.

i don't usually take medicine but i have a hirrible toothache. i have vicodin that is a few years old. can i take 1?

Probably: More than likely it's fine to take it, but its always questionable as to how effective it will be.  Expired medication isn't always as effective.  I'd prefer seeing you take non-expired medication.

the medical director in er suggested that my dr increase my vicodin from5/525 to 10.  my dr. disagrees   what should i do?  my pain isn't being managed.

Judgement Call: Dosing narcotics can be difficult and sometimes sensitive topic.  If you have a legitimate reason to be on higher doses of opiates, you can try to make the case logically to your pcp.  If you feel strongly about it and he/she does not agree then you may consider obtaining a second opinion or seeking out a pain specialist.  I would advise first trialing non-narcotic agents first though.

Opiates: I take it that you&#x27;re talking about pain control. Pain control is an art made more difficult by the limited choices we have. With great attention to detail you can find an answer working closely with your physician. Opiates can be rotated. That said one should not rule out any other modality such as physical therapy ; exercise.

Hard to say: If only a screening test was done, without confirmation, no one can say with certainty that is was hydrocodone. If confirmed, and there was no chain-of-custody there is always a small possibility that the specimens were mixed up, either at their source or in the lab.  It can also be a metabolite of another opiate that was consumed, for example, high doses of codeine.

curious..difference by taking 1tab 5/325 vicodin q 4 hrs vs take 1/2 tab q 2hrs? i don't like that feeling with a whole. been on 4yrs since back surg

That's fine: Ultimately there's no difference - you can take 1-2 every 4 hours (maximum) and really can split that up any way you want.  Do not exceed the 3200 mg of acetominophen per day limit unless your doctor OKs it.

In withdrawal?: If the cramps are due to opiate withdrawal, the cramps are not harmful.

how long does ultram last for pain? vicodin is stronger but doesn't seem to work as well or last as long. is ultram safer

6-8 hours.: Half of the medicine is gone from your body in 6-8 hours.  Medication is generally prescribed every 6 hours.  Vicodin is stronger and half of the medication is gone in 3-5 hours.  Historically tramadol has been considered safer.  There is some controversy right now on it's safety, but it has been out for 15 years already.  In general 325 or less acetaminophen on hydrocodone tabs are preferable.
