Post/repair: This repair is in a very slow healing area, and b/c of the associated problems with healing, extra effort is placed on multiple use of sutures. The "hard mass" is tissue reaction to the types of sutures used. The soreness will last for four to six weeks, post-op.

Ask the surgeon: Ask the surgeon who performed the procedure, or acquire your medical records from him/her and take them to another GYN MD who specializes in Urogynecology and ask for a 2nd opinion Consultation after he/she reads the operative report and all records.

i had a very large rectocele repaired during march.  the doctor told me my bladder is perfect, no need for vaginal repair. after the op, i experienced vaginal incontinence.  this morning i could feel my bladder outside my vagina. is that normal?

Rectocele: To repair a rectocele you must have had an operation in the vagina. This will not help with bladder control or urine continence. I think you need to consult your gynaecologist again.You might need an anterior repair of the vaginal wall or TVT.The rectocele was found at the posterior wall.

does anyone have any successful 4 degree vaginal tear repairs?

Yes: Obviously, the more extensive (i.e. Higher degree) the vaginal tear, the harder it is to repair and recover but with modern surgical techniques, it should be possible.  In fact, these techniques are so successful that we almost never see the sequelae of these injuries such as fistulas (openings between the rectum and vaginal or between the bladder and vagina) in this country any more.

what causes an infection and breakdown of vaginal / perennial repair following a vaginal delivery?

Several: The most common culprit of this type of infection is skin bacteria already present on the vulva. Vaginal bacteria can also play a role. Blood flow from resolution of the placental site contribute "food" to bacteria. Bacteria from the rectum may also contribute. There is one serious infection that can occur in this situation so vigilance & close monitoring by you & your doctor is important.

Surgical technique: It is unlikely to cause harm but your surgeon will know exactly what was done and what techniques were used. It is likely they will want you to refrain at least for a brief period of time. Since we do not know exactly what was done surgically it is best we do not give a recommendation only 3 days after. I am glad your surgical recovery is going so well though.

Need more info: A cone biopsy isn't a vaginal wall repair, but perhaps your physician did both.  Ask, or get a copy of the operative note and see what is described.  Billing for a procedure not done would be fraud and most physicians are honest, so it may have been done or could be a coding error.

Options: Depending on the cause of the fistula, additional diagnoses, age and health of the patient your surgeon will discuss the options. Have friends and family with you to help with this discussion. Usually diagnostic tests colonoscopy, ct scan of abdomen and pelvis are done before final surgical planning.
