You're OK: If you mean that the surgery was 4 weeks ago and there is some vaginal spotting of old blood this is normal and expected. As long as you have no fever, worsening pain, urinary symptoms, or vomiting, you should do fine.

Find out why: The first step is to find out why the odor exists in the first place. Odors can arise from bacterial vaginal infections, infected vaginal cuffs, or retained surgical instruments or sponges post surgery. Have your GYN surgeon examine the area soon  to determine the cause and hopefully get treated.

PCOS: PCOS is a condition where ovulation is essentially not occurring, but cysts form in the ovaries secondary to incomplete (failed) ovulation efforts. It usually causes very abnormal to absent menstrual cycles.  There are agents to assist with ovulation, and others to produce withdrawal bleeding which prevents some effects of long term PCOS.  Hysterectomy is not the only solution.

Bacterial vaginosis?: The main cause of strong vaginal odor, sometimes with discharge, is bacterial vaginosis, an overgrowth of mostly normal vaginal bacteria. Hysterectomy might trigger it. The doctor who did your hysterectomy undoubtedly has a lot of experiecne with BV. Contact her for advice on diagnosis and treatment. It is unlikely you can accurately sort this out without professional evaluation. Good luck!

UTI/STD: Your symptoms, as described, sound like a Urinary Tract Infection or Sexually Transmitted Disease. Are you sexually active? Regardless, you will need to be evaluated by a physician and have your urine tested and cultured. You should also be screened for STDs (HIV, GC/Chlamydia, RPR). An empiric antibiotic could be started depending on your answers to additional SHx questions. See a doctor ASAP.

Bacterial Vaginosis: Knowing you are not preg or sex active helps. This could be a UTI and if so needs to be dx'd and tx't. It could also be bacterial vaginosis, this is an overgrowth of atypical vaginal bacteria which when symptomatic produce a vaginal odor (often described as fishy) and discharge. It is treated with abx &amp; prognosis is good usually causes no probs unless preg. Main other concern would be yeast infect

Need exam: It is not normal to continue seeing brown or bloody discharge after a hysterectomy. If the hysterectomy was done less than six weeks ago then it could be normal. However, if it was more than six weeks ago then there are certain things like polyps or new lesions of the vagina can cause that kind of spotting. You need to see your gynecologist for an examination.
