Most likely: this represents some dehydration.  Ketones are only pathologic ("bad news") when they are noted in moderate to large amounts in uncontrolled INSULIN dependent DIABETES.

is it normal for urine dip to show 0.2 eu/dl urobilinogen? what about showing moderate leukocytes? thanks.

Possible UTI: Moderate Leukocytosis could be Urinary tract infection if Nitritr and or Leukocyte Esterase positive. To make sure Liver Function is ok get a LFT or Liver Function test mild urobilinogen elevation is not a big concern as long as Billirubin level is normal!

did urine dip at work and it is positive for nitrates and protein 3+ everything else is normal.  what does this mean?

Urine dipstick: Nitrite: many bacteria produce enzyme nitrate reductase which confers the ability to convert urinary nitrate to nitrite. Positive test means possible UTI. Next step is urine culture . Protein:most sensitive to albumin in urine but positive protein (trace, 1+, 2+ 3+) may not reliable if urine sample is dilute. Dipstick may not detect albuminuria  of 30 to 300 mg/day. More tests needed. Consult PCP

May be normal ...: Seeing frothy urine sporadically - not consistently -  could be normal for you reflecting urine concentration + what you consume resulting in temporary alteration in certain ingredients forging bubbling. So, if all your check-ups in urine, urine flow, residual urine, blood, etc. show normal or unremarkable findings, you need no to worry further. As to benign polyp, I assume it at bladder neck - ok

Need more info: CRP may be elevated due to any inflammation, e.g. pneumonia, rheumatoid arthritis, and bacterial infections.
Protein in urine is an indication of kidney disease, e.g. glomerulonephritis. Commonest causes of protein in urine are diabetes and high blood pressure. Talk to the doctor who ordered these tests as s/he knows more about your health.

Wrong test site: A urine dip stick picks up white blood cells that are present in urine if  there is an inflammatory process in the urinary tract.  It does so by testing for an enzyme (leukocyte esterase)  present in white cells.  The urinary system is a filter of the blood stream but usually active white cells are very sticky and would not make it from the circulation to the urine. ?Sinus aspirate?

what are the chances of still having a uti/bladder infection if urine dip and culture were all negative? tested 2 times in the same week

No typical infection: If a urine dipstick tests for infection and is negative, then there is no infection, and the lab usually won't culture the urine. One can be sure of no infection if a repeat dipstick is negative. If the lab cultures the urine even anyway and gets a negative result, that would be the expected result. One should continue looking for causes of symptoms, other than a typical UTI with typical bacteria.
